All athletes, officials, participants, parents, and spectators attending the swim meet on May 20 are required to execute the MWR Liability Release Form.

The form applies to organized events held on Navy Base Guam such as the swim meet. It is a release of liability, and not a request for base access. All minors and even persons with military base access are required to sign it.

Once signed, please submit to your team president or other organizing official by Tuesday, May 16, 2023. Scanned copies will be accepted.
